This collection will start on 1 July 2021

If you do not want your data to be shared with NHS Digital please register your Type 1 Opt-out with your GP practice by 23 June 2021.

New Patient Registration

Ross Road Medical Centre looks after patients that live within our Maidenhead practice boundary. We always welcome new patients joining the practice.

Due to COVID 19, we will not ask you to attend the surgery to register you may now do this online.

Please complete a pre-registration form and a Health Questionnaire, please ensure you fill the forms correctly and include your NHS number. 

Due to the current COVID 19 Pandemic, if you have registered online you will need to call us to complete registration.

Please note that patients will not be registered with the practice until the required information has been provided correctly. Please allow at least 48 hours for your registration to become active.

Patients are registered with the practice and not an individual doctor.

Change of Personal Details

Please ensure to update us on any changes to your personal details on a regular basis. This allows us to stay in contact with you should the need arise. Please inform our reception team when you are next in the practice of any changes by completing a change of detail form.
